Description: Revised Standard Flood Hazard Determination Form

Attached is a copy of the Federal Emergency Management Agency's (FEMA) revised Standard Flood Hazard Determination form, "FEMA Form 81-93, Dec 05," which replaces "FEMA Form 81-93, Oct 02." Either form may be used prior to July 1, 2006, when use of the revised form becomes mandatory.

The only changes made were minor revisions to the instructions. The new form is available on the FEMA Web site at [http://www.fema.gov/library/viewRecord.do?id=1394]. Camera-ready copies are available from the FEMA Distribution Center at 1-800-480-2520. The form may be used in a printed, computerized, or electronic manner.
