Please provide the following information for retail lending operations as of the close of business (DATE), unless otherwise indicated. Information in an electronic format is preferred. If submitting hard copies, please prominently mark any information/documentation that is to be returned to the bank.

Our intent is to request information that can be easily obtained. If you find that the information is not readily available or requires significant effort on your part to prepare, please contact us prior to compiling the data.

Please note that this list is not all-inclusive, and that we may request additional items during the course of our examination.
